1 SUPINE HEAD IN MIDLINE

SUGGESTED FRAMING

INSTRUCTIONS:

Hold the head for 5s in midline position and turn it completely from one side to the other.


2 RAISE THE HEAD

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Supine

Raise the head and maintain the raised position for 5s.


3 FLEX HIP AND KNEE

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Supine

Flex the hip and knee>90° by raising the foot from the mat.


4 DORSIFLEXION OF THE FOOT

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Supine, Leg supported by examiner

From plantar flexion, dorsiflexion the foot to at least 90° in relation to the leg


5 RAISE THE HAND TO OPPOSITE SHOULDER

INSTRUCTIONS:

Starting Position: Supine

Raise the hand and move it to the opposite shoulder


6 RAISE PELVIS AND SPINE

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Supine, lower limbs half ­flexed, knee caps at the zenith, and feet resting on the mat.

Raise the pelvis; the lumbar spine, the pelvis and the thighs are aligned and the feet slightly apart.


7 TURN OVER AND FREE ARMS

Starting Position: Supine

Turn over into prone and free both upper limbs from under the body


8 SIT UP WITHOUT ARMS

Starting Position: Supine

Without upper limb support, sit up


9 SITTING WITHOUT ARM SUPPORT

Starting Position: Seated on the mat

Without upper limb support, maintain the seated position for 5s and then maintain contact between the 2 hands in “prayer position” for 5s


10 TOUCH TENNIS BALL

Starting Position: Seated on the mat, the tennis ball placed in front of the subject

Without upper limb support, lean forward, touch the ball, and sit back again


11 STAND FROM SITTING

Stand up without upper limb support


12 SIT FROM STANDING

INSTRUCTIONS:

Starting Position: Standing.

Without upper limb support, sit down on the chair with the feet slightly apart

Starting Position: Seated on the Chair

Without upper limb support or leaning against the back of the chair, maintain the seated position for 5s, with the head and trunk in midline position


14 RAISE HEAD ALIGNMENT

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, head in flexion.

Raise the head from the flexed position, have the head stay aligned throughout the movement and is maintained and raised in midline


15 PLACE HANDS ON HEAD

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, forearms on the table; but not elbows.

Place both hands on top of the head at the same time while the head and trunk remain in midline position


16 REACH FOR PENCIL

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, pencil on the table

Without moving the trunk, reach the pencil with 1 hand, forearm and hand off the table with the elbow in full extension at the end of the movement


17 PICK UP 10 COINS IN 20 SECONDS

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, 10 coins on the table.

Successively pick up and hold 10 coins in 1 hand during the 20s period.


18 ONE FINGER RING CD

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, 1 finger placed in the center of the fixed compact disk.

Go around the edge of the compact disk with 1 finger without contact of the hand with the table.


19 DRAW LOOPS

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, pencil on the table.

Pick up the pencil and draw a continuous series of loops inside the frame and over its full length touching the top and bottom line of the frame.


20 TEAR FOLDED PAPER

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, holding the sheet of paper.

Tear the sheet of paper folded in 4, beginning from the fold edge.


21 PICK UP THE BALL AND PRONATE HAND

Starting Position: Seated on the chair or wheelchair, tennis ball on the table.

Pick up the ball and holding the ball turn the hand over completely.


22 TOUCH SQUARES ON DIAGRAM

Starting Position: Seated on the chair or wheelchair, 1 finger placed in the center of the diagram.

Raise the finger and place it successively on the squares of the diagram without touching the lines.


23 PLACE FOREARMS/HANDS ON TABLE

INSTRUCTIONS:

Starting Position: Seated on the chair or wheelchair, upper limbs along the trunk.

Place 2 forearms and/or hands on the table at the same time.


24 STAND UP FROM CHAIR WITHOUT ARM SUPPORT

INSTRUCTIONS:

Starting Position: Seated on the chair.

Stand up without upper limb support and with the feet slightly apart.


25 STANDING WITHOUT SUPPORT

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Standing with upper limb supported.

Release the support and maintain a standing position for 5s with the feet slightly apart, the head, trunk, and limbs in the midline position.


26 RAISE FOOT

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Standing with upper limb support on equipment.

Without upper limb support, raise the foot for 10s.


27 TOUCH FLOOR WITH 1 HAND

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Standing.

Without support, touch the floor with 1 hand and stand up again.


28 10 STEPS FORWARD ON HEELS

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Standing without support.

Take 10 steps forward on both heels.


29 10 STEPS FORWARD IN A LINE

SUGGESTED FRAMING

INSTRUCTIONS:

Starting Position: Standing without support.

Take 10 steps forward on a line.


30 RUN

INSTRUCTIONS:

Starting Position: Standing without support.

Run for 10m.


31 HOP 10 TIMES IN PLACE

Starting Position: Standing on 1 foot without support.

Hop 10 times in place.


32 SQUAT TWO TIMES

Without upper limb support, squat and get up twice in a row.
